Life Program!

Free Healthy Lifestyle Program

Funded by the Victorian Government · Managed by Diabetes Victoria

Life! is a free, evidence-based healthy lifestyle program that supports eligible participants to improve eating habits, increase physical activity, and manage stress.

The program is delivered by accredited health professionals, including Accredited Practising Dietitians, and aims to help eligible participants reduce their risk of developing type 2 diabetes, heart disease, and stroke.

The Life! program is available to eligible Victorians and selected approved non-Victorian postcodes.

Please note: Life! is a prevention program and is not available to people living with diabetes.

 

Program Structure

  • 7 sessions delivered over 12 months
  • Practical, education-focused lifestyle support
  • Evidence-based and aligned with national prevention guidelines

 

Delivery Options

Participants may complete the Life! program through one of the following options:


Group Program (Face-to-Face or Online)
Small groups of 8–10 participants delivered in person or online (Zoom or Teams) by an accredited health professional.
• Online sessions: ~45 minutes
• Face-to-face sessions: 1.5–2 hours


Telephone Health Coaching (THC)
Seven individual telephone sessions with an accredited health professional, offering flexibility in time and location.


CALD-Specific Life! Program
Delivered by trained bilingual facilitators in Arabic, Chinese, and Vietnamese, with culturally adapted resources.
The program is also available in plain English.

 

Who Is the Life! Program For?

The Life! program is suitable for adults who:

  • Are at risk of developing type 2 diabetes, heart disease, or stroke
  • Meet eligibility criteria based on risk assessment and postcode
  • Are not currently living with diabetes

Aged Care Nutrition Support

Dietetic Care for Older Adults

At Diala Murr Dietary Clinic, we provide evidence-based dietetic services for older adults, delivered by Accredited Practising Dietitians (APDs). Our aged care nutrition support is tailored to the individual’s medical history, nutritional needs, preferences, and care setting, and is aligned with aged care and clinical nutrition guidelines.

 

Our Aged Care Services May Include

  • Nutrition assessment and review
  • Identification and management of malnutrition risk
  • Meal planning support to meet nutritional requirements
  • Texture-modified diets (e.g. soft, minced, pureed)
  • Nutrition support for chronic conditions
  • Weight monitoring and dietary reviews
  • Education and support for carers and care staff



Services may be provided for individuals:

  • Living independently
  • Receiving Home Care Packages (HCP)
  • Living in residential aged care facilities

  • Home Care Packages
  • Residential aged care facilities
  • Private referrals

NDIS Nutrition Support

Dietetic Services for NDIS Participants

We provide dietetic services for NDIS participants where nutrition support is related to a participant’s disability, functional needs, or health goals and is funded under their NDIS plan. All services are delivered by Accredited Practising Dietitians (APDs) and are provided in line with NDIS Practice Standards.


NDIS Dietetic Services May Include

·       Nutrition assessments and reports

·       Support with meal planning and food routines

·       Nutrition support related to disability-associated needs

·       Assistance with special diets where clinically indicated

·       Education for participants, families, carers, and support workers

·       Progress reviews and required documentation

Services are individualised and aligned with the participant’s NDIS goals and funding category.
